This 39 y/o female presented to the ER with a new onset seizure. A CT of the head was performed. Due to the findings on CT, MRI of the head, before and after gadolinium, was obtained. Selected images from these studies demonstrate findings consistent with sinusitis and a left frontal lobe abscess with adjacent subdural empyema. Neurosurgical drainage yielded the diagnosis of Streptococcus milleri abscess and subdural empyema.
