



This 37 y/o male presented with a focal swelling in the mid portion of the left lower leg. Selected images from his MRI scan and intraoperative localization images shown above demonstrate a cortical lesion of bone. It has well defined, circumscribed, slightly lobulated borders with surrrounding periosteal new bone formation. It was excised en toto surgically. Pathologic evaluation, including a consultation from the AFIP (Armed Forces Institute of Pathology), yielded the diagnosis of intracortical diaphyseal chondromyxoid fibroma of the tibia. Our pathologists, as well as those at the AFIP (Armed Forces Institute of Pathology), commented that the intracortical location is "quite unusual".
