This 71 y/o male was receiving treatment for Chronic Lymphocytic Leukemia for over one year and presented with a complaint of left hip pain. Plain films, bone scan, and MRI of the hip were performed and representative images from those studies are shown above. There is diffusely abnormal signal in the osseous structures consistent with CLL involvement of the marrow. There are additional focal areas of abnormal signal in the left femoral head. These focal findings were felt to be the result of avascular necrosis possibly related to steroid therapy. Due to unrelenting pain, surgery on the hip was performed. Avascular necrosis and extensive CLL involvement of the femoral head was found.
