This 74 y/o male underwent repair of an abdominal aortic aneurysm. Post operatively he developed increasing renal failure with a slow rise in serum creatnine observed over several days. A MAG-3 renal scan was performed. Serial posterior analog images at 2 minute intervals and delayed static images in three projections shown above demonstrate urinary extravasation from the left side and decreased uptake on the right side. Subsequent CT (selected images shown above) and retrograde study (not shown) confirmed the diagnosis of urinary extravasation, which was likely due to ureteral damage at the time of the abdominal aortic aneurysm repair. Incidental note was made on the CT of thinning of the right renal cortex explaining the diminished right sided function on the renal scan.
