Shown below is how to attach wire to a punch block. First remove the cover exposing your three wires. Next lay the wire over the female connector and push-down with the punch tool. It strips, cuts and connects with one motion!

Here is an EASY way to strap the grounds together on the channels you want to phantom-power is shown below. Use the screws that hold the jacks in place to run your ground wire from channel to channel.
